宣誓書および学生登録票について
About the Pledge and the Student Registration Form
「学生登録票 入力画面」で必要事項を入力して印刷し，「宣誓書」の該当欄に記入のうえ、新入生ガイダンスの際に学籍番号を確認・記入し、提出してください。
「宣誓書」と「学生登録票」の様式は，Ａ４用紙１枚で印刷できるようになっています。
デバイスによってはＡ４用紙１枚に印刷できない場合があるため、パソコンでの入力・印刷を推奨します。
もし２ページ目（白紙）が表示された場合は、１ページ目のみ印刷してください。
Please fill in the required fields on the Student Registration Form Input Page, print it out, and complete the relevant section of the Pledge.
At the new student guidance session, confirm and write your student ID number, then submit the documents.
The forms “Pledge” and “Student Registration Form” are designed to be printed on a single A4 sheet.
Depending on your device, the forms may not fit exactly on one page, so we recommend completing and printing them using a computer.
If a second (blank) page appears, please print only the first page.
Submission Procedure
- Step１「学生登録票」入力
「学生登録票入力画面」で必要事項を入力し、印刷します。
Enter required information on the Student Registration Form Input Page and print it out. - Step２「宣誓書」記入
「年月日」と「署名」欄に自署してください。
Please write your signature and the date in the designated fields on the Pledge form. - Step３新入生ガイダンスで提出
学籍番号を確認し、記入のうえ提出してください。
At the new student guidance session, confirm your student ID number, write it on the form, and submit the documents.
宣誓書 Pledge
入学者は，学長に対して宣誓書を提出する必要があります。該当欄に自署のうえ，提出してください。
All newly enrolled students are required to submit a pledge addressed to the President.
Please sign your name in the designated field and submit the form.
学生登録票 Student Registration Form
大学からの重要事項の連絡・発信用等として利用しますので，正確に入力してください。
This form will be used by the university to send and receive important information.
Please fill it out accurately.
学生登録票 注意事項
Notes on Completing the Student Registration Form
1 本人の氏名，生年月日は戸籍どおり(外国人は住民票どおり)正確に記入してください。
Please write your full name and date of birth exactly as they appear on your family register (for international students, as shown on your residence certificate).
2 ローマ字氏名は，英文証明書にそのまま印字されますので，パスポートに記載された通りのスペルで入力してください。なお，「姓の全て」及び「名の頭文字」は大文字としてください。
Enter your name in Roman letters exactly as it appears on your passport, as it will be printed on your English certificates. Use uppercase letters for the entire family name and the initial letter of the given name.
3 「現住所・Eメールアドレス・電話番号」については，大学からの大切なお知らせ，災害時の安否確認を行う際等に使用しますので，必ず連絡のとれる連絡先を最後まで正確に入力してください。
For your current address, e-mail address, and telephone number, enter accurate and up-to-date contact information that can reliably reach you. This information will be used for important notices from the university and to confirm your safety in case of an emergency.
4 高校卒業後の学歴を有する者は，最終出身学校名・学部・学科等(中退を含む。)まで記入してください。
If you have completed any educational program after high school, please indicate the name of the last school attended (including if you withdrew), as well as the faculty and department.
5 大学からの重要なお知らせ，授業料関係書類や成績表などの郵便物や災害時の安否確認を行う際に使用しますので，正確に入力してください。
Since this information will be used for official university communications, such as tuition-related documents, grade reports, and emergency contact purposes, please ensure that all entries are accurate.
6 在学中に，改姓・改名，現住所変更，保護者等の住所変更等があった場合は，速やかに身上異動・住所変更届を，所属学部又は研究科の担当係に提出してください。
If there are any changes during your enrollment—such as a change of name, address, or your guardian’s address—please promptly submit a Notification of Personal or Address Change to the administrative office of your faculty or graduate school.
7 この学生登録票に記載された個人情報については，個人情報保護法等を遵守の上，適切に取り扱うこととし，在学中において，授業料関係書類の送付，広報誌等資料の送付など本学から連絡(発信)する場合のほか，教学上の名簿作成，修学指導，大学運営や教育活動のために利用します。また，個人が特定されない形で学術研究のために提供することがあります。
We treat personal information in a strictly confidential manner in accordance with the law and university policy concerning the protection of personal information. We will utilize your personal information to contact you regarding tuition fees, and send you course-related documents. In addition to being used to send you PR information and other materials, your information will also be used in the drawing up of student registration lists and for other necessary processes related to the university’s operation and educational activities. In addition, this information may be utilized for academic research in a way that does not identify individuals.
